Senior developers often pride themselves on their ability to spot issues through experience and intuition. This expertise is valuable, but it comes with a hidden cost.

They enable better mentorship. Junior developers learn what to look for by following the same verification steps as senior developers.
They provide audit trails. When security questions arise, you can demonstrate that specific checks were performed.
Security vulnerabilities carry business costs that extend far beyond the technical damage.
There's the direct cost of incident response. The investigation, the fix, the deployment, the monitoring.
There's the trust cost. Users expect their data to be protected. Breaches damage relationships that took years to build.
There's the regulatory cost. Many industries face legal obligations around data protection. Security failures can trigger compliance issues.
There's the opportunity cost. Every hour spent responding to a security incident is an hour not spent building value for users.
Our eight-month vulnerability was fortunate in one respect: we discovered it internally before any malicious exploitation occurred. Not every team gets that lucky.

The vulnerability would never have reached production.
Code reviews serve a purpose that transcends catching syntax errors and debating style preferences. They're a critical defense layer protecting users and business value from technical failures.
But that protection only works when reviews are comprehensive, not just careful. When they're structured, not just thoughtful.
Our eight-month security gap taught us that expertise without structure creates coverage gaps. That assumptions about what others have checked create blind spots. That the absence of visible problems doesn't guarantee the absence of actual problems.
A single checklist item would have prevented this issue. That's not a commentary on developer skill. It's a recognition that human attention needs structure to achieve comprehensive coverage.
The question isn't whether your team conducts code reviews. The question is whether your reviews systematically verify the things that matter most.
Because somewhere in your codebase, there might be a missing validation step. And the longer it stays missing, the more confident you become that everything is fine.
